Integrity Family Care is a patient portal that helps patients and families access their medical records, communicate with their healthcare team, and find information about services and events. In this article, we'll show you how to login to your Patient Portal account.
How to login to Integrity Family Care Patient Portal
To login to Integrity Family Care Patient Portal, follow these steps:
1. Navigate to www.integrityfamilycare.com and enter your patient's unique Patient ID number located on the back of their medical worker ID card or on the registration card they received when they first enrolled in the program.
2. Click on "Login."
3. Enter your patient's unique Patient ID number and click on "Log In."
4. You will be prompted to create a password. Keep this password confidential and do not share it with anyone.
5. Once you have logged in, you will be able to access all of your patient's records and information related to their care.
How to update your profile
Integrity Family Care Patient Portal users can update their profile information by clicking on the "My Profile" tab at the top of the page. Once in your profile, you can update your name, email address, and other personal information. You can also add a photo and blog URL.

How to create a new account
If you are not already a patient of Integrity Family Care, you will first need to create an account. Once you have created your account, you will be able to access their Patient Portal.
To create a new account, please follow these simple steps:
1) Log in to your account on their website. If you do not have an account, click the "Create an Account" link at the top of the page.
2) Click on the "My Info" tab and enter your name, email address, and password. Make sure that your email address is correct as this will be used to log in to the Patient Portal.
3) Click on the "Patient Portal" tab and enter your birthdate (mm/dd/yyyy). This information is necessary so that we can personalize the Patient Portal for you. Your date of birth will also appear on your medical record.
4) Click on the "Create New Account" button. You will now be taken to a form where you can enter additional personal information about yourself. This information will not be shared with any other party other than their team at Integrity Family Care. Once

If you have registered with Integrity Family Care, you can add or update your contact information by logging into the Patient Portal. How to view your medical records
If you have an account with Integrity Family Care, you can view your medical records through their Patient Portal. To access the Patient Portal, follow these steps:
1) From any web browser, type www.integrityfamilycare.com into the address bar.
2) Click the "Integrity Family Care" logo in the upper left corner of the page.
3) On the main page, click "View Your Record."
4) Complete the fields on the screen and click "Submit." Your record will be displayed in a new window.
How to make a payment
Integrity Family Care Patient Portal offers a variety of ways to pay for your care. Here's how to make a payment using their online portal:
1. Log in to the Patient Portal.
2. Click on the "My Account" tab.
3. Under "Payment Methods," select "Make a Payment."
4. Fill out the required information, and click "Submit."
5. Your payment will be processed and you'll receive an email confirmation once the transaction is complete.
How to access your appointment calendar
If you are a registered patient of Integrity Family Care and have not already created an appointment calendar, please follow these instructions to create an account and access your appointment calendar.
To create an account, click the “Register for an Account” link on the homepage of their website. This will take you to a sign-in page where you will be asked for your name, email address, and password. Once you have logged in, click the “My Appointments” link on the left-hand side of the homepage. On this page, you will see a grid with all of your appointments that have been created through their system. To view or edit any of your appointments, just click on the blue “Edit” button next to the appointment that you want to view or edit. If you would like to cancel or reschedule an appointment that is currently scheduled, just click on the “Cancel” button next to the appointment and we will take care of getting it moved for you.
How to report a problem
If you experience a problem with the Integrity Family Care Patient Portal, you can report it by logging in and going to the "Report a Problem" page. There, you will be able to provide information about your issue, including a description of what happened, what you were doing when the problem occurred, and any screenshots or videos you may have.
