How to Log In
To login to the Patient Portal, please follow these steps:
1. From any computer in the Hilltown Community Health Center network, open the website: www.hilltownhealth.org
2. On the left-hand side of the screen, under "About Hilltown Community Health Center," click on "Patient Portal."
3. Enter your Patient Portal user name and password in the appropriate fields and click "Log In."
4. You will be taken to a page where you can view your medical history and current medications.
How to Change Your Password
To change your password, first click on the "My Account" link at the top of the homepage. You will then be taken to the "Account Settings" page. On this page, you will see a link to "Change Password." Click on this link to enter your current password and new password in the appropriate fields.
