How to login to the Heart Institute Patient Portal
The Heart Institute Patient Portal is an online tool that can be used by patients to keep track of their health and medical information. To log in, click on the "Login" link at the top of the page.
How to search for a patient
To search for a patient, click "Patients" on the main menu of the Heart Institute Patient Portal. This will open a list of all patients currently registered with the Heart Institute. To find a specific patient, enter their name in the search bar at the top of the list and press enter. The Patient Portal will return a list of all documents associated with that patient, including their medical records, appointment history, and more.
How to create an account
If you are a new patient, or have never used the Heart Institute Patient Portal before, you will need to create an account. The session will take around 10 minutes to complete and once it has finished, you will be able to use the portal to access your information and make any changes you wish.
To create an account:
1. Click on the ‘Create an Account’ link located at the top-left of the Patient Portal home screen.
2. Fill out the required information and click on the ‘Create Account’ button.
3. You will be taken to a confirmation page which you can check to ensure that your details have been entered correctly. If everything is correct, click on the ‘Create Account’ button to finish setting up your account.
4. You will now be taken to the main Patient Portal home screen where you can start using all of its features!
How to manage your account
If you have created an account with the Heart Institute, please follow these instructions to login:
1. Click on the “Log In” button in the top right corner of the homepage.
2. Enter your email address and password in the appropriate fields and click on the “Log In” button.
3. If you have not already done so, be sure to create a user profile by clicking on the “User Profile” link on the left side of the homepage. This will allow you to manage your account more easily and keep track of your health information.
How to make a request
If you are a patient at the Heart Institute and need to make a request, the following steps will help you get started.
To make a request at the Heart Institute, please login to their patient portal. Once you have logged in, click on the “Request a Service” link in the main menu. This will take you to their request form. Here, you will need to provide as much information about your request as possible. They will then contact you to discuss your request further. How to cancel a request
If you need to cancel a request, please follow these steps:
- Click on the "Cancel Request" link on the patient portal home screen.
- Follow the prompts to cancel your request.
