2. Click on "Patients" on the left-hand side of the screen.
3. Click on "Login" in the top right corner of the screen.
4. Enter your login information and click on "Sign In."
5. If you have a password, enter it and click on "OK."
6. You will now be taken to the main patient portal page.
7. On this page, you will see a list of your current medications and their doses, as well as any lab test results that have been sent to us from the lab. You can also access your appointment calendar and contact information for all of their offices.

If you have a complaint, or if you need to file a complaint, please use the Haydee Docasar Patient Portal. The Patient Portal is easy to use and can be found at:

1) Log in with your patient ID and password.
2) On the left hand side of the screen, click on "My Account."
3) On the "My Account" page, click on "Complaints & Inquiries."
4) Under "Complaints & Inquiries," click on "File a Complaint."
5) Fill out the form and click on "Submit."
6) A confirmation message will be sent to your email address. Please print or save this message for future reference.
