Starting in 2019, all Medicare patients will be required to have a login ID and password in order to access their medical records. If you're a Grayhawk Medical patient, make sure you know how to login and use your new portal!
How to login to Grayhawk Medical Patient Portal
Logging In to Grayhawk Medical Patient Portal can be done in a few simple steps.
To begin, you will need to create an account on the portal. To do this, click on the "Sign In" link located in the top right corner of the main page. After clicking on the link, you will be taken to a login form.
Enter your username and password (which you created when you created your account) and click on the "Sign In" button.
You will now be taken to the main patient portal page. From here, you can access all of your patient records, as well as view information about your health care team.

How to find your login credentials
If you have not already done so, please create an account on the Grayhawk Medical Patient Portal. :
Once you have logged in, please click on the "My Account" link located in the top left corner of the portal.
In the "My Account" page, you will find your login credentials under the "My Profile" tab. Please use these credentials to log in to your account and access your personal information.

How to use the Patient Portal
If you are an existing patient, please login below. If you are a new patient, please click the "Create Account" link to create your account and then login below.
If you are not a patient, please click the "Register" link to register as a new patient and then login below.

How to print out your medical records
If you have a Grayhawk Medical Patient Portal account, you can print out your medical records by following these steps: Log in to your account on the portal. Click the "My Account" tab. Select "View My Medical Records." To print a single record, click the "Print" button next to the record you want to print. To print all of your records at once, click the "Print All" button.
How to change your password
If you have forgotten your password, please follow these steps to change it:
1. Log in to the Grayhawk Medical Patient Portal.
2. Click on the "Account" tab at the top of the page.
3. In the "My Account" section, click on "Forgotten Password?"
4. Enter your email address and password in the fields provided, and click on "Update."
5. If you need assistance resetting your password, please contact their customer service team at 1-866-967-8686 or [email protected].
How to report a problem with the Patient Portal
If you are having trouble logging into the Patient Portal, please follow these steps:
1. Try to reset your password by clicking on the "Forgot your Password?" link in the login form on the homepage. If you have difficulty resetting your password, please email us at [email protected] and we will help you get back into the portal.
2. If you have not been able to reset your password, please email us at [email protected] and we will help you contact customer service for help logging in to the Patient Portal.
3. If you still cannot log in, please submit a problem report by clicking on the "Report a Problem" link on the main menu of the Patient Portal. This will take you to a form where you can provide more information about your issue and we will address it as soon as possible.
