Gonzaba Patient Portal Login
When you first login to the Gonzaba Patient Portal, you will be prompted to set up your password. After you have entered your password, click on the "Log In" button in the top right corner of the screen.
In order to log in to the portal, you will need your patient number and password. Your patient number can be found on your hospital identification card or on the back of your insurance card. Your password can be either the last four digits of your patient number or a word that is at least six characters long.

My Account
If you have not already, please create a user account on the Gonzaba Patient Portal. Once you have created an account, follow these steps to login:
1. From your desktop or laptop computer, open the Gonzaba Patient Portal.
2. In the top left corner of the portal, click on “My Account” (see image below).
3. In the My Account window that opens, enter your Username and Password (see image below).
4. Click on “Log In” in the top right corner of the My Account window.
5. You will be prompted to review and accept their Terms of Use (see image below). Click on “Yes, I Accept” to continue.

Health History
If you’re new to the Gonzaba Patient Portal, we recommend starting by logging in using your Patient ID and password. Once you’ve logged in, you will be able to view your health history and make personalized health recommendations. Below are instructions on how to login and access your health history:
1) Click on the “Login” button at the top of the page.
2)Enter your Patient ID and password.
3)Click on “Health History” to view your current health status and make personalized recommendations.
Medications
Gonzaba Patient Portal is a secure online tool that allows patients and caregivers to manage their medications. To login, visit: https://gonzaba.com/patient-portal/. Once logged in, you will be able to access your account, view your medications, and track your medication history.
Appointments
If you have been referred to the Gonzaba Patient Portal by your doctor, you will need to login first. Here are the steps to follow:
1. Go to the Gonzaba Patient Portal website (https://patientportal.gonzabahealthcare.com/).
2. Click on the "Login" button in the upper right corner.
3. Enter your name and email address in the appropriate fields, and click on "Sign In."
4. If you have a password, enter it in the "Password" field and click on "OK."
5. If you do not have a password, click on "Create a New Account." In the "First Name" field, type your first name; in the "Last Name" field, type your last name; in the "Email Address" field, type your email address; and in the "Phone Number" field, type your phone number. Click on "Next." In the "Gender" field, select either male or female; in the "Birthdate" field, type your birthdate; and in the "Zip Code" field, type your zip code. Click on "Create Account." Billing & Payments
If you are a Gonzaba patient and have not yet created a patient portal account, please follow these instructions to create an account. Once you have created your patient portal account, you can login to it by following these steps:
1. From any computer in the world, go to www.gonzaba.com and click on "My Account."
2. In the My Account section, click on "Patients."
3. Click on "Create Profile" near the top of the Patients page.
4. Complete the Create Profile form with your personal information and click on "Next."
5. You will be asked to select a health plan from their list of plans. If you are not currently enrolled in a Gonzaba health plan, you will need to do so before continuing. After selecting a health plan, click on "Next."
6. You will be asked to provide some basic contact information such as your name and email address. After providing this information, click on "Next."
7. You will now be prompted to create a password for your patient portal account. Make sure that this password is unique and easy for you to remember! Click on "Create Password"
Correspondence
The Gonzaba Patient Portal is an online tool that allows patients to manage their health information and communicate with their healthcare providers. To login, visit the patient portal website and enter your username and password. Once you have logged in, you will see the main page of the patient portal. On the left side of the page, you will see a list of sections. The first section is My Health Records. This section includes tools for managing your medical records, including uploading new documents and downloading existing documents. The second section is My Care Plans. This section includes tools for creating care plans, including adding doctors and medications. The third section is My Communication Settings. This section includes tools for setting up communication preferences, such as receiving email notifications about appointments and messages from your doctor. The fourth section is My Account Overview. This section includes a overview of your account information, such as your name, email address, and password. The fifth section is My Account Activity. This section includes a history of all the activities that have been performed on your account, such as submitting forms or requesting care from your healthcare providers.
