If you are a patient at Gifford Medical Center, you can use the Patient Portal to access your health records and make appointments. The Patient Portal requires you to log in using your username and password. Here's how to do it:
1. Go to giffordmedicalcenter.net and click on the Patient Portal link in the navigation bar at the top of the page.
2. On the Patient Portal page, click on Login in the top left corner of the page.
3. Enter your username (the name you use when you sign into Gifford Medical Center online) and your password in the appropriate fields. Click Log In to log in to the Patient Portal.
How to register for Gifford Patient Portal
If you are a new patient, or if you have forgotten your login information, please follow the instructions below.
If you are an existing patient, please click on the "My Account" link on the top right of the home page and enter your username and password to log in.

How to login to Gifford Patient Portal
If you have not already done so, please create an account on the Gifford Patient Portal website by clicking on the "Create Account" link in the menu at the top of the page.
Once you have logged in, follow these steps to login to the portal:
1. Click on the "Login" icon in the top right corner of the main screen.
2. Enter your user name and password in the fields provided and click on "Log In".
3. If you have registered for My Health Record, your login information will be automatically populated in the fields for My Health Record. If you have not registered for My Health Record, please enter your primary health care provider's (PHC) registration number in the "Provider ID" field and click on "Register". Your PHC will then be able to access your medical records through My Health Record.
4. If you are a patient who is registered with a GP or specialist, please enter your login information for your GP or specialist in the "Provider ID" field and click on "Register". Your GP or specialist will then be able to access your medical records through their respective online portal.

How to login:
1. Enter your email address and password in the login form on the home page of Gifford Patient Portal.
2. Click on "Login".
3. In the "Login form" window, enter your name and department. If you have forgotten your password, click on "Forgotten your password?" and enter your email address in the "Request a new password" field. Your new password will be sent to you by email.

Once you have logged in, please follow these instructions to access your personal health history:
1. Choose "My Health History" from the main menu. \ n\ n
2. Click on "My Medications." \ n\ n
3. Under "Active Medications," click on the pencil icon next to the medication you want to view more information about. This will open a new window with more detailed information about that medication. (For example, if you are taking medication for high blood pressure, this window will list the blood pressure readings taken while you were taking that medication.) 4. To view all of your medications, select "All Medications" from the My Health History menu and scroll down to view all of your medications.
My medications
If you are a Gifford patient and need help logging in to the Patient Portal, here is how to do it.
To get started, go to the Patient Portal home page on the Gifford website. On this page, you will see a blue “Log In” button. Click on this button to log in.
Once you have logged in, you will be taken to the “My Medications” page. This page will list all of your medications and their doses. You can see which medications are currently being administered to you and how much of each dose you are receiving.
To add or change a medication dosage, simply click on the name of the medication and then select the “Edit” button. This will open up a new window where you can enter your desired dose. Once you have entered your new dosage, click on the “Save” button to save your changes.
If you need help logging out of the Patient Portal, simply click on the “Log Out” button at the bottom of the screen.
My progress notes
If you are new to the Gifford Patient Portal, or if you have forgotten your login information, please follow these easy steps to get started:
1. Click on the "My progress notes" tab located in the main menu of the portal.
2. Enter your user name and password in the appropriate fields and click on the "Log In" button.
3. You will be redirected to the main page of the portal.
My discharge summary
If you have created a Gifford Patient Portal account, logging in is simple. Simply enter your username and password at the top of the page, and you are ready to go!

If you are a Gifford patient, you can login to the Patient Portal using your Patient ID and Password. If you have forgotten your Patient ID or Password, please follow these instructions to retrieve them:
1) Log in to the Patient Portal using your patient ID and password.
2) Click on "My Account" on the top right corner of the page.
3) Under "My Account Info," click on "Change Password."
4) Enter your current password in the new password field and click on "Update."
5) Enter your new Patient ID in the New Patient ID field and click on "Update."
6) Click on "Log Out" at the bottom of the page.

-In the login form, enter your username and password.
-If you have previously registered for the Patient Portal, enter your registration credentials in the "User Name" and "Password" fields. If you have not registered for the Patient Portal, click on "Register Now".
