To login to the Frederick Primary Care Patient Portal, please follow these steps:
1. In the upper left-hand corner of the website, click on "Login".
2. Enter your name and password in the appropriate boxes and click "Login".
3. You will be taken to a page where you can review your current account settings. Click on "Account Details" to update your contact information or to change your password. You can also view your account history by clicking on "History". How to find your health history
If you are new to Frederick Primary Care, please sign in here. If you are already registered with them, please enter your username and password below.
If you would like to search for your health history, please click on the "My Health History" tab located at the top of the page. Here you will be able to review all of your visits to Frederick Primary Care and see a summary of your health conditions.
If you would like to view individual diagnoses or treatments that we have treated you for, please click on the "View Diagnoses" or "View Treatments" tabs, respectively.

How to view your health records
If you are a Frederick primary care patient and have an account on the patient portal, you can view your health records by following these steps:
1. Log in to your patient portal account.
2. Click on “My Health Record” from the top menu.
3. Scan the QR code to open your medical record in a new window.
4. Click on any of the health records listed to view more information about that record, including medication allergies and more.
How to request a copy of your health records
If you would like a copy of your health records, please contact Frederick Primary Care. You can request a copy by filling out the form below. They will then send you a copy of your health records.
How to make a complaint
If you have a problem with your care at Frederick Primary Care, you can make a complaint. To do this, you need to first login to the patient portal. Once you're logged in, follow these steps:
1. Click on the "My Account" tab on the top of the page.
2. On the My Account page, scroll down to the "Complaints" section and click on it.
3. On the Complaints page, click on the "Make a complaint" link in the top right corner.
4. Complete the online form and submit it.
