1. Go to fpa-women.org and sign in using your username and password.
2. Click on "My Profile" in the upper right corner of the page.
3. On the My Profile page, under "My Health Record," click on "Access My Health Record."
4. On the Access My Health Record page, enter your email address and password in the appropriate fields, and click on "Log In."
5. You will be taken to a screen that asks you to select a health care provider from the list of participating providers. If you have more than one health care provider, select the provider that you would like to use as your default provider. After selecting a health care provider, click on "Next."
6. On the next page, you will be asked to provide your name, date of birth, gender, insurer name and number (if applicable), contact information (phone number and email
How to Report a Health Issue
If you are experiencing a health issue and would like to report it to the FPA Women's Health Patient Portal, please follow these steps:
1. Click the "Report a Health Issue" link on the main page of the FPA Women's Health Patient Portal.
2. You will be prompted to enter your login information.
3. If you are a current user, you will be taken to the "My Account" page. If you are not a current user, you will be prompted to create an account.
4. On the "My Account" page, click on "Login."
5. Enter your login information and click on the "Login" button.
6. You will be brought to the "Reporting a Health Issue" page.
7. Under "What type of health issue do you have?" select "Health Issue I am experiencing."
8. Under "What makes this health issue concerning to you?" enter as much information as possible about your health situation in the text box below. Be sure to include any relevant symptoms, including if they occur only during certain activities or times, how long they have been occurring, and whether they have changed in
