How to login to the Patient Portal
If you are a patient, or family member of a patient, and you need to login to the Fmh Caretrack Patient Portal, follow these simple steps:
1. Log in to your personal account on the website.
2. Click on the "Patients" tab on the left side of the page.
3. On the "Patient Portal" tab, click on the "Login" button in the top right corner.
4. Enter your user name and password and click on "Log In."
5. You will be redirected to the Patient Portal main screen.
How to use the Patient Portal
If you are a new patient or have not used the Patient Portal in the past, please follow these instructions to login:
First, go to www.fmh.org and sign in with your MyFMH account.
Next, click on the "My FMH" icon in the upper right corner of the page. This will take you to the My FMH home page. On the left side of this page is a section called "My FMH Settings." Click on the "Patient Portal" link in this section.
Now you will be directed to a new page that looks like this:
Please enter your MyFMH Username and Password if you have already registered for an account or created one. If not, please click on “Create an Account” and follow the simple instructions. After you have entered your information and clicked on “submit”, you will be taken back to the main Patient Portal page where you can start using it!
