How to login to Florida Hospital Provider Portal
If you are a Florida Hospital Provider and need to access your account or make changes to your account, you can login using the following information:
Provider Name:
Your provider name will be the same as your Florida Hospital ID.\
Provider ID:
Your provider ID is an 8-digit number that is found on the front of your identification card.\
Username:
Your username is the same as your email address.\
Password:
Your password is the same as your provider ID.\\
How to view medical records
If you are a Florida Hospital Provider and have an account with FLH Portal, you can view your medical records by logging in to your account. To login, go to FLH Portal and click on the “Provider Login” button in the upper right-hand corner of the screen. Enter your Provider ID number and password, and you will be logged in to your account. Once you are logged in, you will see a list of all of your medical records.
How to change your password
If you have forgotten your password, follow these steps to change it:
1. Log in to the Florida Hospital Provider Portal.
2. Click on "My Account" in the top left corner of the page.
3. Under "My Profile," click on "Login."
4. Enter your username and password, and click "Log In."
5. On the "Login Status" page, click on the "Forgot Password?" link next to your username.
6. If you have not already done so, enter your new password in the "New Password" field and click "Save."
7. Click on the "Log Out" link at the bottom of the page to log out of the Florida Hospital Provider Portal.

How to get more information about Florida Hospital Provider Portal
If you are a Florida Hospital provider, you can use the Provider Portal to find information about your hospital, view your electronic health records, and manage your billing. To sign in to the Provider Portal, follow these steps:
1. Go to the Florida Hospital Provider Portal homepage at ProviderPortal.FloridaHospital.com.
2. Click the Login link in the upper left corner of the page.
3. Enter your username and password in the appropriate fields and click Login.
4. You will be taken to a page that lists all of your hospital accounts.
5. On the Accounts page, you will see an account for each hospital that you have created with Provider Portal.
6. Click on an account to view its details:
-Your name: The name that will appear on your hospital bills and in Medline search results. This name can only be changed by contactingProviderPortal customer services (1-855-THERAPIST).
-Account type: Select one of the following options: Provider Account (for providers who have completed registration with ProviderPortal) or Beneficiary Account (for patients or families who have registered with ProviderPortal as their healthcare
