With the Family Living Institute Patient Portal, you have everything you need to keep track of your health and wellness. Here's how to login and get started:
1. Go to familylivinginstitute.org and sign in.
2. Click on "Patient Portal".
3. You will see a list of tabs on the left side of the screen.
4. Click on "My Health & Wellness".
5. You will now be in the "Settings" tab.
6. Under "My Health & Wellness", click on "Login".
7. Type in your password and click on "Log In".
8. You will now see the "My Health & Wellness" tab with all your information at the top of the screen!
How to login to the Family Living Institute Patient Portal
To login to the Family Living Institute Patient Portal, follow these steps:
1. Open the website at familyliving.org.
2. Click on the Patient Portal icon in the upper right corner of the screen.
3. Enter your user name and password in the fields provided and click on the Login button.
4. You will be taken to a page where you can review your current log-in status, as well as change your password and update your personal information.
How to add or update personal information
If you are a Family Living Institute patient and have not already done so, you will need to create an account on their Patient Portal. To create an account, click the "Create Account" link on the Patient Portal home page. You will need to provide your name, email address, and hospital identification number (HIN) in order to create your account. If you have not yet registered with Family Living Institute, please do so now by clicking the "Register" link on the Patient Portal home page. After you have created your account, please follow these instructions to add or update personal information:
1. Click the "My Account" link on the Patient Portal home page.
2. Click the "Personal Information" link in the My Account section of the Patient Portal.
3. Click the "Add or Update Personal Information" link in the Personal Information section of the Patient Portal.
4. In the Add or Update Personal Information form, enter your name and email address in the appropriate fields and click the "Submit" button.
5. You will receive an email notification confirming that your personal information has been updated on their Patient Portal.
How to manage your health records
If you have any questions about the Family Living Institute Patient Portal, or need to manage your health records, follow these steps:
1. Log in to the Patient Portal.
2. Click on "My Profile."
3. Click on "Health Record Management" in the left column.
4. In the "Health Record Management" area, click on "Manage My Health Records."
5. On the "Manage My Health Records" page, click on the blue "Log In" button to log in to your account.
6. Enter your username and password and click on the green "Log In" button to log in to your account.
7. Underneath your name, click on the blue "My Health Records" link to view and manage your health records.
How to receive notifications about important updates and changes
If you would like to be notified about important updates and changes, please login to the patient portal. Once you have logged in, select "Notifications" from the menu on the left-hand side of the page. You will then be able to select the type of notification you would like to receive: email, text message, or push notification.
How to contact Family Living Institute
When you are ready to login to the Patient Portal, please follow these simple steps:
1. Click on the link in their email that you received after registering for the Patient Portal.
2. Type your user name and password into the login form and click on the "Login" button.
3. You will be prompted to review and agree to their Terms of Use before continuing. After you have reviewed and agreed to their terms of use, click on the "Accept" button.
4. You will be taken to the Patient Portal home page. On this page, you will see a blue bar across the top of the page that says "Log In." The text below this blue bar is your user name and password. Please enter these values into the login form below this blue bar and click on the "Log In" button.
5. You are now logged in to the Patient Portal!
How to unsubscribe from email notifications
If you no longer wish to receive email notifications from the Family Living Institute, you may unsubscribe at any time by following these simple steps:
1) Log in to the Patient Portal.
2) Under "My Account," click on "Email Preferences."
3) On the "Email Preferences" page, select the "Unsubscribe from Email Notifications" check box.
