1. Go to www.familyhealthpartners.org/patient-portal/.
2. Click on the "Log In" button in the upper right corner of the homepage.
3. Enter your email address and password in the appropriate fields and click on the "Log In" button.
4. You will be taken to the main Patient Portal page. On this page, you will see a list of all of your records and folders, as well as the ability to add new folders and records. You can also print out your health record or save it to your computer.

1. Go to www.familyhealthcarepartners.org and click on "Login".
2. Enter your username and password and click on "Log In".
3. On the left-hand side, under "My Account", click on "My Family Healthcare Partners".
4. Click on the "Communication" tab and under "My Communication Settings", click on "Configure Communication Settings for My Family Healthcare Partners".
5. Check the box next to each family healthcare partner you would like to communicate with, and then enter their email addresses in the appropriate fields. Click on the "Save Settings" button.
