Once you have created an account, you will be able to access all of the tools and information that Fairway Family Medicine has to offer.
How to manage your account
If you are a new patient, you will need to create an account on the Fairway Family Medicine Patient Portal. Logging in is easy - just follow these simple steps:
1. Click on the "Login" button at the top of any page on the Patient Portal. This will take you to the login screen.
2. Enter your email address and password in the appropriate fields, and click on the "Log In" button. You will be redirected to the home page of the Patient Portal.
3. If you have previously logged in, simply click on the "My Account" link located in the top right corner of any page on the Patient Portal. Your account information will be displayed here.
4. If you have not yet registered for an account, please do so now by clicking on the "Register" link located in the top right corner of any page on the Patient Portal. You will need to provide your name and email address in order to create an account. Once your account has been created, you can login using your email address and password.

If you have ever used a health care website, you are likely familiar with the patient portal. A patient portal is a centralized location where patients can access their health information and manage their health records.

If you have a problem with your visit to their office, please don't hesitate to file a complaint. The following information will help you file your complaint in the most efficient way possible.
First, log in to their patient portal using your username and password. If you are having trouble logging in, please contact them at 541-694-7777 and we will be happy to help you out. After you login, click on the "My Account" tab located on the top right of the page. This will take you to the page where you can select the type of complaint that you would like to file.
Once you have selected the type of complaint that you would like to file, follow these simple steps:
1) Write down what happened during your visit so that you can provide as much detail as possible when describing your issue.
2) If applicable, take pictures or videos of any relevant documents or physical evidence.
3) Complete the contact information form and make sure to include your name, address, and phone number so that we may contact you directly if needed.
