Englewood Family Physicians offers a patient portal that allows patients to view their medications, refill prescriptions and track their health information in one place. To access the patient portal, patients must first login. Here are instructions on how to do so:
To login to the patient portal, patients must first be registered with the practice. After registering, patients will be automatically redirected to the patient portal. To login, patients must enter their name and password in the appropriate fields and click the "Log In" button. If a patient has forgotten their password, they can request a new password by clicking on the "Forgot Your Password?" link on the home page of the portal. Patients can also reset their password by entering their old password and new password in the "Reset Password" field and clicking on the "Reset Password" button.
Once logged in, patients will be able to view their current medications, refill prescriptions, view their health history and more. The patient portal is dynamic, meaning that it will update automatically as new information becomes available. In addition, all of Englewood Family Physicians' clinical documents (e.g., discharge summaries) are available online through the patient portal.
How to book an appointment
Englewood Family Physicians Patient Portal offers a variety of ways to book appointments. You can use their online appointment scheduler, call us at (844) 438-3232, or walk in to one of their locations.
To use their online appointment scheduler, click the "View Calendar" button on the home page and select the date and time you want to book an appointment. Our calendar will show available appointments for the next 7 days. To make an appointment, click the "Book Appointment" button and fill out the required information. You can also choose to receive a notification email about your appointment when you register. If you would like to be contacted about a future availability for that same time slot, please indicate this in the "Comments" field when registering.
