How to login to Duke Patient Portal
If you are a Duke patient and have not already registered with the Patient Portal, please visit www.patientportal.duke.edu to register and create your user ID and password. Once you have registered and logged in, follow these steps to login:
Click on the "Login" link on the main navigation bar at the top of the page. Enter your User ID (which is your first and last name) and Password (which is the same as your User ID). Click on the "Log In" button. You will be taken to a page where you can review your recent activity, add new entries, or change your password.
How to find your health information
If you are a Duke patient, you can access your health information through their Patient Portal. You can find the Patient Portal by going to www.dukehealth.org and clicking on “MyHealth” in the top left corner of the page. In the MyHealth page, click on “Log In” in the top right corner.
To log in, enter your email address and password in the appropriate boxes and click on “Log In”. If you have forgotten your password, please contact them at [email protected].
Once you have logged in, you will be taken to the “MyHealth” page. On this page, you will see a list of all of your appointments and health services that we have provided to you. You can also find information about your insurance coverage and how we are charged for their services. You can also view your medical records and messages from their team about your care.

How to dispute a medical decision
If you feel that you have been wronged by a medical decision, there are steps that you can take to dispute the decision. The following is a step-by-step guide on how to login to the Duke Patient Portal and dispute a medical decision.
1) Log in to the Duke Patient Portal.
2) Click on "My Health Record" in the top right corner of the screen.
3) On the My Health Record page, click on "Dispute A Medical Decision."
4) Fill out the form with your information and explain why you believe that the medical decision was wrong.
5) If you are satisfied with your explanation, click on "Submit Dispute."
6) If you are not satisfied with your explanation, you can submit a new dispute or continue with your original dispute.
How to get help from the Duke Patient Portal
If you are having trouble logging in to the Duke Patient Portal, follow these instructions:
1. Log in to your Duke account (either on the web or through your smartphone app).
2. Click the MyHealth tab at the top of the page.
3. Under MyHealth Tools, click the Duke Patient Portal link.
4. Enter your Patient ID and password in the appropriate fields, and click Log In.
5. If you have not yet registered for a patient portal account, enter your full name and email address in the Account Registration fields and click Register Now.
