Accessing my personal health information
The Due West Family Medicine Patient Portal is a great way to stay organized and connected with your health care team. Once you have registered for the Patient Portal, you will be able to access all of your personal health information, including your medical history and medications.
To log into the Patient Portal, follow these simple steps:
1. Go to www.duewestfamilymedicine.com/patient-portal/.
2. Click on the blue "Log In" button in the top right corner of the page.
3. Enter your username and password in the appropriate fields and click "Log In."
4. You will now be able to view all of your personal health information, including your medical history and medications.
Requesting a copy of my health record
If you are a patient of Due West Family Medicine, you can request a copy of your health record from their Patient Portal. This can be done in one of two ways:
1. From the Patient Portal home page, click on "Request Your Health Record." This will take you to the Request Form page. On this page, you will need to provide your contact information and the reason for wanting a copy of your health record. You will also be required to agree to the terms and conditions of accessing your health record.
2. You can also request a copy of your health record by calling their office at (505) 827-8111. Our office staff will be happy to assist you with this process.
Checking my Vital Statistics
If you have registered with the Due West Family Medicine Patient Portal, you can view your vital statistics by logging in. To login, enter your User ID and password in the appropriate fields on the Patient Portal home page. If you have not registered with the Patient Portal, or if you have forgotten your User ID or password, please contact their office at (205) 444-3400.
Requesting a referral to another doctor
If you are looking for a referral to another doctor, their patient portal can help you do just that. Here are the steps to login and request a referral:
1. Login to the patient portal by entering your unique patient identifier (PIN) and password.
2. On the main page, click on the "Referring Physicians" tab.
3. Click on "Request Referral" under the "Referring Physicians" header.
4. In the "Referral Request Form" window, complete all required fields and click on "Submit."
5. A confirmation message will be displayed indicating that your referral request has been submitted.

Applying for insurance through the Patient Portal
If you are uninsured or have limited coverage through your employer, you may be able to get health insurance through the Patient Portal. To apply through the Patient Portal, you will need your social security number and date of birth. You can also use the Patient Portal to view your health insurance records, update your contact information, and more.
