How to login to the Dr Sullivan Patient Portal
To login to the Dr Sullivan Patient Portal, please follow these steps:
1. Navigate to http://www.dr-sullivan.com/patient-portal/.
2. Click on the “Login” button in the upper right-hand corner of the page.
3. Enter your email address and password into the respective fields and click on the “Login” button.
4. You will now be prompted to select a user name and password. If you have not created an account yet, enter your first name and last initial in the user name field and click on the “Create Account” button.
5. Once you have logged in, you will be presented with a screen displaying all of your recent visits and clinical notes.
How to find your medical records
If you have forgotten your login information or need to update it, please follow these steps:
1. Click on the "login" tab at the top of the page.
2. Enter your name and email address in the appropriate fields, and click "submit."
3. A confirmation email will be sent to you with your login information. If you do not see this email, please check your spam folder. If you still cannot find it, please contact their office at 215-862-4000.

What if I have a question that isn’t answered on the website?
If you have a question that is not answered on the website, you can try contacting the office directly. You can find the contact information for their office on their website or by looking on their patient portal. Once you have contacted us, please let us know what question you had and whether or not we were able to provide an answer.
What if I need to change my scheduled appointment?
If you need to change your scheduled appointment, please follow the instructions below.
If you have already made the changes, please login to the patient portal and follow the instructions on how to reschedule your appointment.
To login to the patient portal:
1) Click on "My Account" in the top right corner of their website.
2) Enter your email address and password in the appropriate fields and click "Log In."
3) If you are a new patient, click on "New Patient" in the top left corner of their website and follow the instructions.
4) If you are an existing patient, click on "My Appointments" in the top left corner of their website and follow the instructions.
5) Click on "My Doctor" in the top right corner of their website.
6) Click on "View Appointments With This Doctor."
7) If you need to change your scheduled appointment, click on "Appointments" in the top right corner of their website and follow the instructions.
What if I need to speak with a doctor outside of regular office hours?
If you need to speak with a doctor outside of regular office hours, you can use the Dr. Sullivan Patient Portal. The Patient Portal is a secure website where you can communicate with your doctor and receive medical advice and treatment.
To login to the Patient Portal, follow these steps:
1. Go to www.drsullivan.com and login using your username and password.
2. Click on the "Patient Portal" link in the left-hand navigation bar.
3. Enter your patient identification number (PID) in the "Login ID" box and click on the "Log In" button.
4. You will be prompted to enter your medical history information. If you have a prescription for medication, you will also be asked to provide this information. After you have completed this information, click on the "Next" button.
5. You will now be asked to choose a doctor from the list of available doctors who are registered with the Patient Portal. Click on the "Select Doctor" button and then select a doctor from the list who is available for appointments outside of regular office hours.

Can I get my records sent to me electronically?
Yes, you can have your records sent to you electronically. To do this, please login to the website and navigate to "My Account" on the left-hand side. On the My Account page, select "My Records" from the drop-down menu and then click on the "Upload Records" button. You will need to provide your name and email address, and then select the type of record you would like to have sent to you: chart, letter, or summary. Once you have selected your record type, click on the "Submit" button. Your record will be uploaded and will be available for you to view online immediately.
Can I opt out of receiving marketing materials
Yes, you can opt out of receiving marketing materials from the Dr Sullivan Patient Portal. To do this, sign in to the portal and select "Settings" from the top navigation bar. From here, you can choose to receive only communications pertaining to your account or all communications from the portal.
