How to sign up for health insurance through Patient Portal
If you're looking for a way to get health insurance, you might want to consider using the Patient Portal. This website is designed to help patients find and sign up for coverage. Here are the steps you need to take to sign up:
1. Go to the Patient Portal website.
2. Click the "Sign In" button in the upper-right corner of the homepage.
3. Enter your account name and password.
4. Click the "My Account" tab at the top of the page.
5. Under "My Coverage," click "Get Started."
6. On the next page, click "Create My Account."
7. Fill out the form with your personal information, including your date of birth and Social Security number. Note: If you don't have a health insurance plan yet, you'll need to choose a plan from one of the options on this page. How to log in to Patient Portal
If you have been redirected to the Patient Portal after logging in to your account, please follow these steps:
1. Click on the "Forgot your password?" link at the top right of the Patient Portal screen.
2. Enter your email address and click on the "Forgot your password?" link again.
3. You will receive an email with a new password. Enter this password in the login form on the Patient Portal screen and click on the "Log in" button.
How to manage your health records through Patient Portal
If you are a registered patient of Lisa Wasemiller Smith, you can use Patient Portal to manage your health records. To login, first go to www.lwws.com/patientportal and enter the patient ID number that is printed on your medical record or on your hospital bracelet. If you do not have a patient ID number, call the office. After logging in, you will see a list of your health records. You can access your health records and make changes to them by clicking on the links in the left column. You can also print out your health record or send it to another person by clicking on the link in the right column.

If you're new to the Patient Portal, or if you're looking for a refresher, these tips will help you get started and stay healthy through the system.
To login to your account, click the "Login" button on the top right corner of any page in the Patient Portal. If you're having trouble logging in, please see their troubleshooting guide.
To see your account summary, including recent appointments, medications, lab results and more, click the "My Account" button on the top left corner of any page. You can also access this information by clicking on your name in the header of any page.
To add or update your personal information, such as your address or email address, click the "Edit My Profile" button on the top right corner of any page. You can also change this information by clicking on your name in the header of any page. Finally, you can delete your profile by clicking on the "Delete My Profile" button at the bottom of any page.
