The first step is to login to the Dignity Health Provider Portal and enter your ID and password. Once you have logged in, click on the "Report a Health Information Breach" link in the menu bar.
To begin the breach notification process, you will need to provide some basic information about the incident: what happened, when it happened, who was involved, and what information was compromised.
