How to create an account
To create an account on the Dekalb Health Patient Portal, please follow these steps:
1. Click "Create an Account" located in the top right corner of the homepage.
2. Fill out the required information and click "Create Account."
3. You will be redirected to a page to sign in with your existing account or create a new one. If you are new to the Dekalb Health Patient Portal, please enter your email address and password and click "Sign In."
4. After you have logged in, you will see the "My Account" page. From here, you can view your profile information, register for notifications, and more.
How to manage your health information
If you're new to the Dekalb Health Patient Portal, now is a great time to get started! Here are some helpful tips on how to login and manage your health information.
First, create an account by clicking the "sign up" link in the main menu of the Patient Portal. Once you've registered, you'll be able to access all of your information in one place. To login, enter your username and password in the login fields at the top of any page. You can also sign in using your My Dekalb account.
Once you're logged in, you'll see a list of all of your appointments and messages from doctors and nurses. You can also view your health history and see what medications you're taking. You can also contact your doctor or nurse if you have any questions or concerns about your health.
How to get a copy of your health records
If you are a Dekalb Health patient, you can request a copy of your health records by logging in to the Patient Portal. To login, click here. Once you are logged in, go to My Account and select Request a Copy of My Health Records. You will be asked to provide your name, date of birth, and health insurance number. Your health records will be emailed to you within 10 business days.
How to report a security incident
If you believe that you have experienced a security incident, please follow these instructions to report it:
1. Go to the Dekalb Health Patient Portal and sign in.
2. Click on the "Settings" button in the top-right corner of the screen.
3. Under "Security," click on "Report Security Incident."

How to change your password
If you have forgotten your Dekalb Health password, or need to change it, follow these steps:

4. You will be sent an email with a link that you can use to log in to your account. Click on the link in the email to login to your account.

1. Log in to the Dekalb Health Patient Portal.
2. Click on the "Email Notifications" link in the navigation bar on the left side of the page.
3. On the "Email Notifications Settings" page, scroll down and click on the checkbox next to "Subscribe to Blog Posts and Health Updates."
