How to login to Crittenton Patient Portal
If you would like to login to the Crittenton Patient Portal, you will need your patient number and PIN. To find out your patient number, please click here. Your PIN can be found on the back of your health insurance card. If you have forgotten your PIN, please contact Crittenton at 1-866-603-9578. Once you have logged in, you will be able to view all of your account information, including your health history, medications, and appointment schedule.
How to find your Crittenton identification number
If you have a question about your Crittenton account or need to login, you can find your identification number on the Patient Portal. Just go to the Patient Portal and login with your email address and password. You'll see your identification number on the top right corner of the screen.
How to change your password
If you haven't done so already, please change your password. You can do this by clicking on the "My Account" link at the top of the page and then clicking on "Change Password." Be sure to remember the new password!
How to report a medical emergency
If you experience a medical emergency, the best way to ensure your safety is to immediately report the situation to your doctor or 911. If you are unable to reach your doctor or 911, the Crittenton Patient Portal can help you communicate with the hospital.
To login to the Crittenton Patient Portal, click here. Once you are logged in, click on the Patients tab and select My Medical Emergencies. From here, you can select a medical emergency that occurred recently or choose an emergency that is more important to you and fill out all of the necessary information.
