How to login to the Corvallis Patient Portal
If you are a patient, your first step is to create an account. If you are not a patient and need to access the Patient Portal, please see the following instructions.
Once you have created an account, you will be able to login using the following information:
-Your patient number
-Your email address
-The password that you entered when you created your account
How to find your account information
If you have not already done so, please create an account on the Corvallis Patient Portal. Once you have created your account, you will be able to access your account information and settings by logging in at www.corvallispatientportal.org/login.
To log in to your account, enter your username (e.g. "JohnDoe") and password (e.g. "password"). If you do not remember your password, please visit their Password Recovery Page to find help recovering it.
Once you have logged in, you can access the following items from your account:
-Your profile
-Your medical history
-Your health information preferences
-Your appointment calendar
-The messages and notes you have sent or received through the portal
How to change your password
If you have forgotten your password, or if you have changed your password and need to reset it, follow these steps:
1. Click "Log In" in the upper left corner of the home screen.
2. Enter your email address, and then click "Log In."
3. Click "My Account" in the upper right corner of the screen.
4. Click "Change Password" in the lower left corner of the My Account screen.
5. Enter your current password, and then click "Change Password."
6. You will be asked to confirm your new password. Type it in and click "Confirm Password."
7. You will be taken to a confirmation page where you can click "Finish" to log out of your account or "Cancel" to stay logged in.
How to make a complaint
If you have a complaint about your care at Corvallis HealthCare, you can make a formal complaint through the Patient Portal. Please read the following instructions carefully before making a complaint:
To make a complaint, follow these steps:
1) Log in to the Patient Portal.
2) Click on "My Care" in the top left corner of the screen.
3) In the "My Care" section, click on "Make a Complaint."
4) In the "Make a Complaint" form, please provide as much information as you can about your complaint. This includes: Date of incident/problem Date of service Time of incident/problem Detail of what happened (e.g., why did you feel that your care was not adequate?) Please note: You may be asked to provide additional information or documents to support your complaint.
5) When you are finished filling out the form, click on "Submit." Your complaint will be processed and someone from their team will contact you to discuss your concerns.
