1. Go to communitylink.com/ Provider Portal.
2. Enter your login information into the form on the screen. (You will need your business name, email address, and password.)
3. Click the Login button to finish setting up your account.
How to login to the Provider Portal
If you are a Community Link Inc provider and have not already registered for an account with the Provider Portal, you can do so now by clicking on the Register link in the upper right corner of the Provider Portal home page. Once registered, you will be able to access your account information, as well as manage your provider profile and contact information.
To login to the Provider Portal, first ensure that you are logged into your account on the Provider Portal home page. If you are not logged in, click on the Log In link in the upper right corner of the Provider Portal home page and enter your username and password. Once logged in, you will see the Login screen. On this screen, type your provider ID (found on your registration confirmation email) into the text field next to ID and click on the Login button. You will be redirected to a new screen where you can enter your verification code. Once entered, click on the Submit button to return to the Provider Portal home page.

The first thing you need to do is sign in to the Provider Portal. Once you're signed in, you'll see a list of all the community health centers that are affiliated with Community Link Inc.
To find a community health center that's close to you, you'll first want to search for a location using the search bar at the top of the page. You can enter your city or zip code into the box and then click on the "Find Clinics" button. This will display a list of clinics that are located within a certain distance of your location.
If you don't see a clinic near you that's affiliated with Community Link Inc., or if you'd like to view a list of clinics by type (e.g. mental health, pediatric), you can click on the "Clinics by Category" tab at the top of the page. This tab will display a list of clinics that are categorized according to different areas of healthcare.

2. Enter your username and password in the appropriate fields and click "Login."
3. You will now be directed to their calendar page.
4. To make a new appointment, click on the "Book Appointment" button located in the middle of the page and fill out the required information.
5. Once you have completed your appointment information, click on the "Submit Appointment" button located at the bottom of the page and submit your request!
How to pay for services
If you are looking to pay for services through Community Link Inc., you can do so through the provider portal. To login to the provider portal, first go to the website and sign in. Once you have logged in, click on the "Services" tab at the top of the page. Under "Services," you will see a list of all of the services that Community Link Inc. offers. To purchase a service, click on the service that you want to purchase and then fill out the information requested. Once you have completed all of the required fields, click on "Apply." You will then be taken to a confirmation page where you will need to provide your credit card information. If everything goes according to plan, your credit card information will be processed and your service will be automatically billed.
