How to Request a Transcript
If you would like to request a transcript from Penn’s colonial records, you can do so through the Provider Portal. The Provider Portal is a web-based system that allows authorized Penn personnel to access and process requests for transcripts. You must first create an account on the Provider Portal and then complete the required fields in your request. You will need to provide your name, date of birth, email address, and Penn ID number. After you have completed the request form, you will need to send it to the Penn Registrar’s Office.
How to Request an Audit
If you are a provider who uses Colonial Penn's Provider Portal, you may want to request an audit. To do this, follow these steps:
1. Log in to the Provider Portal.
2. Click on "Services" in the left-hand menu.
3. Click on "Audits."
4. Click on the "Request Audit" button.
5. In the "Request Audit Details" section, provide your name, organization name, and contact information. You will also need to provide your login information for the Provider Portal account you wish to have audited (if you have multiple Provider Portal accounts, specify which one you would like to have audited).
6. In the "Details of Your Request" section, provide any additional information about your request that you feel is important for their staff to know.
7. Click on the "Submit Request" button and wait for a response from us!
How to Report a Violation
