If you're looking for your health record, you can use the Cny Cardiology Patient Portal. To login, first create an account by clicking on the "Create an Account" link on the Patient Portal home page. Next, enter your contact information and select a password. You'll then be prompted to provide your date of birth. Finally, click on the "Login" button to continue.
If you don't have an account yet, you can create one by clicking on the "Sign In" link in the upper right-hand corner of the Patient Portal home page. Once you've logged in, you'll see your personal health record on the left-hand side of the screen. You can access additional health information by clicking on any of the tabs at the top of the page.
