To use the patient portal, first you will need to create an account. After creating your account, you will need to login. To login, you will need your patient number and password. Once you have logged in, you will be able to access all of your information on the patient portal.
How to login to the Clear Lake Family Physicians Patient Portal
If you are a Clear Lake Family Physicians patient, you can login to their Patient Portal using your email address and password. The Patient Portal is a secure online resource that allows you to interact with your doctor and other patients in a variety of ways. You can access the Patient Portal from any computer with internet access.
To login to the Patient Portal, follow these steps:
1) Log in to your email account. If you do not have an email account, you can create one at www.clearlakefamilyphysicians.com.
2) Enter your email address and password into the fields at the top of the Patient Portal screen.
3) Click “Login” to log in to the Patient Portal. Your email address and password will be automatically saved for future use.
How to find your account information
If you have already registered with the Clear Lake Family Physicians Patient Portal, log in by clicking on the "Login" link at the top of the home page. If you have not yet registered, click on the "Register" link at the top of the home page to create an account. How to change your password
If you are a new patient, or have not logged in for some time, please follow these instructions to change your password:
1. Go to their Patient Portal at www.clfp.com and click on the link in the top right corner of the homepage.
2. On the login screen, enter your email address and password and click on the Log In button.
3. On the My Account screen, under Settings, click on Change Password.
4. Enter your current password in the New Password field and type a new password in the New Password (again) field. Click on Change Password.
5. Click on Log Out to finish setting your password.
6. If you have forgotten your password, please call their office at (281) 346-4000 and we will reset it for you over the phone.
How to update your contact information
If you have a new address, or have changed your name since your last visit, now is the time to update your contact information on the Patient Portal. First, log in to the Patient Portal and click on My Account in the top right corner. Then, under Profile, click on Update Contact Info. On the next page, you will need to enter your new address and/or name. Once you have updated your contact information, please save your profile by clicking on the Save Profile button at the bottom of the page. How to unsubscribe from notifications
If you no longer wish to receive notifications from the Clear Lake Family Physicians Patient Portal, you can unsubscribe by following these simple steps:
1. Navigate to the Patient Portal home page and click on the "Notifications" link in the top left corner of the page.
2. On the "Notifications" page, locate and click on the "Unsubscribe" link next to the patient portal notification you would like to unsubscribe from.
3. Follow the prompts on the Unsubscribe page to complete your unsubscription.
