4. Under “My Complaints,” find the complaint you want to make, and click on it.
5. On the right side of the screen, fill out the relevant information: what happened, when it happened, who was involved, and your contact information (including email address). You can also attach a photo or video if you have one handy.

2. On the My Services page, under Health Services, you will see all of the services offered by Chmc.
