Chkd Patient Portal is a patient portal that helps you manage your medical records, appointments, and medication. To login, follow these steps:
1.First, create an account on the Chkd Patient Portal website.
2.Second, enter your email address and password into the login screen.

How to create an account

To create an account:

3. You will be redirected to a confirmation page. Please click on the "Create Account" button again to complete your registration.

How to login
If you are a CHKD patient, you can login to their Patient Portal to access your medical records, learn more about your care and share your experiences with others. To login, follow these steps:
1. Go to the Patient Portal website at http://www.chkd.org/patient-portal/.
2. Click the "Login" link in the upper left corner of the homepage.
3. Enter your name and email address in the appropriate fields and click "Sign In."
4. You will be prompted to create a password. Make sure you remember this password! If you have lost your password, click "Forgot Your Password?" on the login page and follow the instructions there.
How to update your personal information
If you have not done so already, please complete the Chkd Patient Portal login form to update your personal information. All fields must be populated in order for you to login. If you are having difficulty logging in, please email us at [email protected] with your name and patient number, and we will help you out as soon as possible.

3. Enter your name and email address and click on ‘Create My Account’.
4. You will be prompted to enter your password again. If you have forgotten your password, click on ‘Forgotten Your Password?’ and follow the instructions.
5. Once you have entered your details, click on the ‘Log In’ button to log in to the patient portal.
How to report a health concern
If you have a health concern that you would like to report, you can use the Chkd Patient Portal. The portal allows you to submit your health concern anonymously and receive feedback from healthcare professionals. To login to the portal, follow these instructions:
1. Go to chkd.com and enter your email address in the "Log-in" box on the upper right hand corner of the homepage.
2. Enter your password in the "Password" box and click "Log-in."
3. On the home page of the portal, click on "My Concerns."
4. In the "My Concerns" section, click on "Submit a Health Concern."
5. In the "Submitting a Health Concern" section, select "Anonymous."
6. In the "Details about my health concern" section, provide as much information as possible about your health concern.
7. Click on "Submit."
8. You will receive an email notification about your health concern submission. Read and respond to the feedback that you received from healthcare professionals in the email notification.
