How to login to the Chesapeake Eye Care Patient Portal
If you are a new patient, or have not been logged into the Patient Portal in a while, please follow these instructions to login:
1. Click on the link "Log In" in the upper right corner of the homepage.
2. Enter your username and password, and click Log In.
3. If you have forgotten your password, enter your email address in the "Forgot Password?" field, and click Reset Password. You will then be sent an email with instructions on how to reset your password.
4. Once you have logged in, you will see a list of recent appointments on the left side of the page. You can also view your medical history and account details by clicking on the "My Account" link at the top of the page.
How to find your account number
If you have an account with them, you can find your account number by logging in to the patient portal. If you don’t have an account with them, or if you forgot your account number, you can find your account information on their website.
Once you have logged in to the patient portal, click on “My Account” in the top left corner of the screen. You will see a list of all of your current appointments and services. Beneath each appointment or service, you will see your account number and the date that your appointment was scheduled. You can also find this information on their website under “My Account”

How to change your password
If you have forgotten your password, click here to reset it. If you have never logged in to the patient portal before, follow these instructions to create a new account:
-Click on the blue "Log In" button in the upper right corner of the homepage.
-Enter your username and password (you may need to create a new account if you haven't logged in before) and click on "Log In."
-If you have more than one account with Chesapeake Eye Care, select the account you would like to use from the dropdown menu at the top of the page.
-Click on "My Settings" in the upper right corner of the page and make sure that "Remember Me" is checked.
-Click on "My Content" in the left column and select any articles that you would like to view or save. You can also create a new blog post by clicking on "Post a Blog."
How to view your account information
To view your account information, please enter your email address and password in the login form on their website. If you have forgotten your password, please enter your email address and we will send you a link to create a new password.
