Finding a specialist
If you’re looking for a specialist to help with your chronic coronary heart disease (CHD), the University of Michigan Health System has compiled a list of specialists who can provide you with the care you need.
To find a specialist, first try the U-M Health System Primary Care website. This online directory includes contact information for more than 2,500 doctors and other health professionals. You can also use the U-M Health System’s online search tool to find a specialist based on specialty or location.
If you don’t find a specialist in their directory or search tool, you can also ask your doctor if he or she knows of any specialists who treat CHD. And finally, if you still can’t find the right specialist, you can always go to the U-M Health System website and request an evaluation by one of their specialists.
Appointments and bookings
If you are having any problems logging in to the Chcrr Patient Portal, or are experiencing any issues, please follow these steps:
1. Navigate to www.chcrr.org and log in using your username and password.
2. Click on the ‘My Account’ tab located on the top right of the screen.
3. Click on ‘Appointments and bookings’ located under ‘My Account’.
4. Enter your dates of birth and click on the ‘Search’ button. You should now see all of your appointments from the previous 12 months displayed.
5. To make a new appointment, click on the blue ‘New Appointment’ button located next to the appointment you would like to book. This will take you to the Appointment Form which you can fill in with all of the relevant information about your appointment, such as name, date, time and doctor/specialist. Once you have completed the form, click on the ‘Submit’ button to send it off for booking. Prescriptions
If you are a Chcrr patient and need to access your prescription records, follow these instructions:
1. Log in to the Chcrr Patient Portal using your provider login credentials. If you have not yet registered for an account with the Patient Portal, you will need to do so first.
2. Click on the "My Prescriptions" tab on the left-hand side of the page.
3. Under "My Prescriptions," select the prescription that you would like to view details about. The information below the list of prescriptions will show you specific information about that prescription, such as the date it was filled and its pharmacy name.
4. If there are any problems logging in or viewing your prescriptions, please contact your provider's office for assistance.
