The Center for Digestive Health Patient Portal offers patients access to their health records, medication refills, and more. In order to login, you need to provide your patient number and last name.
How to login to the Center For Digestive Health Patient Portal
If you are a patient of the Center for Digestive Health, you can login to the Patient Portal using your provider identification number (PIN) and your email address. To login, click on the “Log In” link at the top of the page. If you have forgotten your PIN or your email address, please contact their office at 860-393-6800.
How to find your medical record
If you're a patient at the Center for Digestive Health, we want to make sure that you have easy access to your medical record. Here's how to find it:
1. Log in to their patient portal using your email address and password. If you don't have an account, sign up now.
2. Click on "My Medical Record." You'll see all of your medical records in one place. You can print or save them as PDFs, or share them with your doctor or family members.

How to request a copy of your medical record
If you would like a copy of your medical record, please follow these steps:
Step One: Log in to the Patient Portal and click on “Request a Copy of Your Medical Record.”
Step Two: Fill out the information requested, including your full name, mailing address, and email address.
Step Three: Click on the “Submit Request” button to send your request. A member of their team will review your request and contact you within 10 business days to let you know whether or not we have received it. If we have not received your request, we will contact you again to update you. Thank you for using their Patient Portal!
How to make a change or correction to your medical record
If you need to make a change or correction to your medical record, you can do so through the Patient Portal. To login, please follow these steps:
1. Log in to the Patient Portal by clicking on the "Login" button on the homepage.
2. Click on the "My Account" tab at the top of the page.
3. Enter your username and password in the appropriate fields and click on the "Login" button.
4. On the My Account page, under "My Medical Record," click on the "Edit My Record" link.
5. Complete the form and click on the "Submit" button.
How to unsubscribe from email notifications from the Center For Digestive Health Patient Portal
If you no longer want to receive email notifications from the Center For Digestive Health Patient Portal, you can unsubscribe by following these steps:
1. Log in to the patient portal.
2. Click on the "Notifications" link in the left-hand navigation panel.
3. In the "Notifications" page, click on the "Unsubscribe" link next to the email address associated with the notification you wish to unsubscribe from.
