2. On the left side of the screen, click on “Patient Portal”.
3. On the Patient Portal screen, click on “Login”.
4. Enter your name and email address, and then click on “Sign In”.

How to request a copy of your health records
To request a copy of your health records, please visit the Centennial Primary Care Patient Portal. After logging in, select "My Health Records" from the menu on the left-hand side. A form will appear which you can complete to request a copy of your health records. You will be notified regarding the status of your request within 7 business days.

3. Select your primary care provider from the list of providers displayed on the next page, and click "Next."
4. If you have more than one primary care provider, select the one you would like to use as your default provider. Click "Next."
5. Click "Update Personal Information" to make any changes to your personal information, such as your mailing address or phone number.
6. Click "Save Changes" to save your changes and return to the main Patient Portal page.
