How to Login to the Cdc Portal
If you are not already logged in to the Cdc Portal, please follow these simple steps:
1. Navigate to cdcportal.cdc.gov and sign in with your user name and password.
2. In the left navigation panel, click on "My Account."
3. Under "My Account," click on "Login."
4. Enter your user name and password, and click on "Log In."
5. If you have an active MyCDC account, your username will be displayed in the "Username" box below the "Log In" button. If you do not have an active MyCDC account, your email address will be displayed in the "Email Address" box below the "Log In" button. Click on either of these boxes to log in using that information. If you do not have an email address associated with your user name, you will need to create a new MyCDC account before logging in to the Cdc Portal using that information.
How to Change Your Password
If you have forgotten your password, or need to change it, you can use the Cdc Portal login page. To login, you will need your username and password. How to Register for an Account
If you would like to create a personal account on the CDC Portal, you will need to first register for an account. To register for an account, please follow these steps:
1. Log in to the CDC Portal at cdcportal.cdc.gov.
2. Click on "My Account" in the top left corner of the page.
3. On the My Account page, you will see a link called "Register New Account." Click on this link to begin the registration process.
4. Complete the registration form by providing your name and e-mail address. Your e-mail address will be used to activate your account and send you important Portal updates and notifications. You will not be able to login until your account has been activated.
5. After registering, please click on the link that says "Create New Account" to create your personal account on the CDC Portal.
How to View your Cdc Report
If you have logged in to the CDC Portal using your Username and Password, you can view your Cdc report by clicking on “My Reports” on the main menu and selecting “CDC Report.” The report will open in a new window and will include all of the data that was collected about you during your visit to the CDC Portal.
How to Request a Copy of Your Cdc Report
If you have forgotten your Cdc Portal login information, or if you need to request a copy of your report, follow these steps:
1. Log in to your Cdc Portal account.
2. Click on "My Reports" from the main menu.
3. Select the report you want to view.
4. On the "Report Details" tab, scroll down to the "Request a Copy" section and enter your contact information.
5. Click on the "Submit Request" button to send your request and receive a response.
