1. Click on the Login link on the main menu of the Provider Portal. This will open the Login screen.
2. Type in your email address and password into the appropriate fields and click on the Log In button. You will be redirected to a confirmation page.
3. If everything looks correct, click on the Log In button and you will be logged in to the Provider Portal.
How to find your Ccs Provider
If you are not already registered with your Ccs Provider, please visit their website to register. Once you have registered, you will be able to login to the provider portal.
To login to the provider portal:
1. Go to the website of your Ccs Provider.
2. Click on the Login link in the upper right corner of the homepage.
3. Enter your username and password and click Login.
4. You will be directed to a page showing your account details.
5. Click on My Account in the top right corner of this page to open the My Account window.
6. In the My Account window, under My Provider Portal, click on Connections to open the Connections window.
7. In the Connections window,click on Connection Settings in the left column to open the Connection Settings window. How to submit a claim
If you have a claim related to a Cotton Carrier Service (CCC) violation, please follow these steps:
1. Go to the CCS Provider Portal and sign in.
2. Click on the "My Claims" link in the top navigation bar.
3. Select the claim you wish to submit.
4. On the "Submit Claim" page, provide the following information:
- The date of the violation.
- Your contact information, including your name, phone number, and email address.
- A description of what happened.
- A statement about whether you are seeking financial compensation for damages or want CCC to take corrective action.
5. Click on the "Submit Claim" button to submit your claim.
How to view a claim history
If you need to view a claim history for a CCS provider, you can do so by logging in to the provider portal.
