Carolina Family Healthcare Patient Portal is a secure web-based portal that allows patients to access their medical records, receive updates on their health and wellness, and sign up for care plans. To login to the Patient Portal, follow these steps:
1. Go to www.carolinafamilyhealth.org and sign in with your Carolina Family Healthcare user ID and password.
2. Click on My Account in the top left corner of the screen.
3. On the My Account page, click on the patient account icon in the right column (see photo below).
4. On the patient account page, click on Login in the top right corner of the screen (see photo below).
5. Enter your user ID and password in the appropriate fields (see photo below), and click Submit.

2. Click on the "My Profile" icon in the top right corner of the page
3. Enter your login ID and password
4. Click on "View My Health Information" to open the Patient Portal
How to make changes to your health information
If you have registered with Carolina Family Healthcare and have an account, you can make changes to your health information by logging in to the Patient Portal. To log in, go to www.carolinafamilyhealthcare.org and click on the "Patient Portal" link in the top navigation bar.
To update your contact information or add a new physician, click on the "My Profile" tab and then on the "Edit My Profile" button. On the "Edit My Profile" page, you will need to enter your login credentials (your user name and password) and select the "Update My Contact Information" check box. You can also update your medical history by clicking on the "My Medical History" tab and then on the "Edit My Medical History" button.
