How to login to the patient portal
To log in to the patient portal, you will need your username and password. To find out your username and password, please click on the "Forgot Your Username?" link on the login screen. You will then be able to enter your username and password to log in. If you have forgotten your username, please click on the "Forgot Your Password?" link on the login screen. You will then be able to enter your email address and create a new password.
How to use the patient portal
If you have not already created an account on the Capstone Family Practice patient portal, you can do so by clicking the link in the header of this page. Once you have logged in, please follow these instructions to access your patient portal:
1. Click on My Account at the top of the page.
2. On the My Account page, under "My Profile," click on "Patient Portal."
3. Enter your email address and password in the appropriate fields, and then click "Log In."
4. You will be directed to a Welcome Page that will explain how to use the patient portal. Please follow the instructions on this page to access your medical records, schedule appointments, and more. Enjoy using the patient portal!
How to report a problem with the patient portal
If you experience any problems logging in to the patient portal, please follow these steps:
1. Click the "Report a problem" link on the top right of the page.
2. Enter your login information and hit submit.
3. A representative from their support team will contact you to help resolve the issue.
How to find information about your health
If you're a Capstone family practice patient, you can use their patient portal to stay connected with your health and schedule appointments. To access the portal, sign in using your Patient ID and password. You can also find information about your health, including medications and insurance benefits, on their website.
How to submit a medical claim
If you are a patient of the Capstone Family Practice, please login to their patient portal. This portal is where you can submit your medical claim online. Please note that all medical claims must be submitted online.
How to find out about upcoming appointments
There is no need to search for information about upcoming appointments on the website. You can view your appointment calendar and make changes to your personal schedule by logging in to the patient portal.
How to login:
1. Click the "Login" link on the navigation bar at the top of the page.
2. Enter your email address and password in the appropriate fields. The password must be at least 8 characters long and include at least one letter and one number. You will be prompted to confirm your password after you enter it.
3. Click the "Log In" button to activate your account and begin using the patient portal.
How to share your health information with your family
There are several ways to share your health information with your family using the Capstone Family Practice Patient Portal. You can choose how you want to share information, and who can see it. You can also manage your health information online, including viewing and editing your medical records.
To share your health information with your family online, sign in to the Patient Portal. On the main menu, click My Health Information. On the left side of the My Health Information screen, select Sharing Options. Under Who Can View This Information, select one or more people who you want to share this information with. If you don't want anyone to see this information, select Hide This Information from All Users. If you want other people in your family to be able to view this information but not edit it, select Allow Other Family Members To View This Information But Not Edit It. If you want other people in your family to be able to view this information and edit it, select Allow Other Family Members To View This Information And Edit It. Click Share My Health Information Now.
