How to Login to the Capital Region Patient Portal
To login to the Capital Region Patient Portal, click the "Login" button on the top right corner of the portal homepage. Enter your username and password, and hit "Log In." If you have not registered for the portal yet, you will be prompted to do so.
How to Create an Account on the Portal
If you are new to the Capital Region Patient Portal, or if you have forgotten your login information, follow these steps to create an account on the portal.
1. Go to the portal homepage and click on “Create an Account” in the upper right corner.
2. Enter your name, email address and password in the appropriate fields and click “Create Account”.
3. You will be redirected to a confirmation page where you can review your information and click “Confirm Account” if you are ready to continue.
4. On the main page of the patient portal, click on “My Account” in the upper right corner.
5. In the My Account section, under “Activities”, click on “Access my medical records”.
6. Click on “Create an account” in the lower right corner of the Access my medical records window and enter your name, email address and password in the appropriate fields. You will then be redirected to a confirmation page where you can review your information and click “Confirm Account” if you are ready to continue.

How to Access your Record
If you need to access your record, first visit their website and sign in. Once you are signed in, click on the "My Account" tab. You will then be able to view your account information, including your account number and password. If you have not yet set up an account, please click on the "Create an Account" button on the left hand side of the screen. After creating your account, please enter your account number and password in the appropriate fields and click on the "Login" button. You will then be able to access all of your records online.
