How to find your health information
If you're not sure where to find your health information, start by looking in your health care provider's office or online. You can also use the Breeze Patient Portal to access your health information.
To login to the Breeze Patient Portal:
1. Go to breezepatientportal.com and sign in.
2. Click on "My Account" on the left-hand side of the screen.
3. Under "My Health Information," click on "Login."
4. Enter your username and password, and click "Log In."
5. You'll now be taken to the My Health Data page. Here you can find all of your health information in one place, including your medical record and medication history.
How to file a complaint
If you have a problem with Breeze Healthcare, you can file a complaint. To file a complaint, go to the Breeze Patient Portal and:
1. If you're using Internet Explorer:
a. Click on the three horizontal lines in the top right corner of the browser window.
b. Select "New Tab."
c. In the "New Tab" window, click on "File."
d. Select "Complaint."
e. Fill out the form and click on "Send."
2. If you're using Firefox:
a. Click on the three horizontal lines in the top right corner of the browser window.
b. Select "Tools."
c. Select "Web Browser."
