Blake Medical Center is excited to announce their new patient portal. With this new system, patients can easily view their health records, schedule appointments, and more. In order to login and begin using the system, follow these simple steps:
1. Complete the registration form on their website by providing your name, email address, and phone number.
2. Once you have registered, you will receive an activation code via email. Enter this code into the login form on their website.
3. You will now be able to access all of the features of their patient portal!
How to login to the Blake Medical Center Patient Portal
If you are a patient at Blake Medical Center, you can use their Patient Portal to access your health information, see your appointment history, and more! To login, follow these instructions:
1. Go to www.blakemedicalcenter.com and log in using your login credentials from the hospital or health system where you received care. If you do not have login credentials from Blake Medical Center, you can create an account by clicking “Create Account” on the homepage.
2. On the left side of the page, click “Patient Portal”.
3. On the Patient Portal home screen, click “My Account”.
4. Enter your name and email address in the appropriate fields, and click “Save My Account”.
5. Click “Log In” to enter your patient ID and password. If you have not registered for online access to your medical records at Blake Medical Center, a registration form will be displayed asking for basic contact information (such as name, date of birth, etc.). Click “Register now” to complete the registration process and gain immediate access to
How to locate the information you need on the Patient Portal
The Patient Portal is the online patient portal for Blake Medical Center. It allows you to access your medical records, track your health information and communicate with their healthcare team.\\
To locate the information you need on the Patient Portal, follow these steps:\\
1. Log in to the Patient Portal. If you have not already done so, register for an account by clicking on the Register for an Account link on the main menu of the Patient Portal.\\
2. On the left-hand side of the Patient Portal, under My Account, click on My Medical Records. This will display all of your medical records that are currently housed on their system.\\
3. If you have questions about any of your medical records, or would like to request that we modify or add information to one of your records, click on Modify Your Medical Record link below My Medical Records and fill out the required fields. You can also access other important information about your medical history by clicking on the links in the header of My Medical Records page.\\

How to file a complaint or request a change on the Patient Portal
If you have a complaint or would like to request a change on the Patient Portal, please follow these easy steps:
1. Log into the Patient Portal.
2. Click on "My Account" in the top navigation bar.
3. Under "Settings & Help," click on "Complaints & Requests."
4. Fill out the form and click "Submit." Your request will be processed within two business days.
How to stay connected with Blake Medical Center
With the Blake Medical Center Patient Portal, you can stay connected with your health care team and track your health status anytime, anywhere. To login, follow these simple steps:
1. Log in to the Patient Portal using your patient ID and password.
2. From the Patient Portal home page, select My Health Record to view your health record.
3. View recent medical appointments and hospital stays by selecting the relevant dates from the calendar on the left side of the page. You can also print or download your health record summary at any time.
4. Request changes to your health information by selecting My Health Record and then Request Changes in the menu on the right side of the screen. Your request will be sent to the appropriate Blake Medical Center personnel for review and approval.
5. Send messages to your care team by selecting My Care Team from the menu on the right side of My Health Record and sending a message directly to a member of your care team.
