If you have forgotten your username or password, please follow these instructions to retrieve them:
Log In to the Patient Portal by clicking on the “Login” link located in the top right corner of every page. Enter your Username and Password, then click on the “Login” button. If you have forgotten your Username, please contact them at (651) 386-5000. If you have forgotten your Password, please enter your Email Address and click on the “Forgot Password?” link. Your Email Address will be sent to you along with instructions on how to reset your Password.
How to add a new doctor or health care provider
Blaine Medical Center offers an online patient portal that allows patients to easily add a new doctor or health care provider. To add a new doctor or health care provider:
1. Click on the "My Account" link on the homepage of the patient portal.
2. Select "Add a New Doctor".
3. Enter the required information and click "Save".
4. The new doctor will be added to the list of providers available on the patient portal.
How to change your password
If you have forgotten your password, or if you want to change it, follow these instructions:
1) Log in to the Blaine Medical Center Patient Portal.
2) Click on "My Account."
3) Under "Account Settings," click on "Password."
4) Enter your old password and new password in the appropriate fields, and click "Update Password."
How to update your contact information
If you have not updated your contact information on the Blaine Medical Center patient portal in at least 6 months, please follow these steps to update your information:
1. Sign into the patient portal using your account ID and password.
2. Click on the "My Personal Info" tab.
3. Under "Contact Info," click on "Update Personal Info."
4. Enter your full name, email address, and phone number.
5. Click on the "Update My Info" button to save your changes.

1. Enter your date of birth in the Date of Birth field.
2. Enter your email address in the Email Address field.
3. Click the Login button.
4. Enter your password in the Password field and click the Login button again.
5. You will be taken to the My Account page. Here you will find important information such as your account type (Patient), primary care physician, contact information, and appointment history.
6. To view or update any of your account information, click on the appropriate link under My Account on the left side of the page.
How to request medical records
If you need copies of your medical records, you can request them through Blaine Medical Center's patient portal. To access the portal, sign in using your credentials and click on "My Account" in the left-hand navigation menu. From there, click on the "Medical Records" tab, and select the "Request Medical Records" button. You will then be prompted to provide some basic information about your health condition, and you will be given a deadline for submitting your request. Once your request is received, a specialist will review it and determine whether or not you are eligible to receive copies of your records.
