Alliance Community Hospital Patient Portal is a patient portal that allows patients to access their medical records, receive health alerts and updates, and book appointments online. In thisarticle, we will guide you through the steps necessary to sign in to the Patient Portal and start using it.
How to login to the patient portal
If you have not already done so, please create a user account on the patient portal by clicking on “Create Account” in the top right corner of the home screen. Log in using your email address and password. Once you are logged in, you will see the “My Settings” page. In this page, click on the “Activities” tab. You can now select the type of activity that you would like to view: hospital visits or surgeries.
To view your hospital visits, click on “View Hospital Visits” in the Activities tab. On this page, you will be able to see a list of all of your hospital visits, including the date, time, location and patient name. You can also view a detailed summary of each visit by clicking on the “View Summary” button.
To view your surgeries, click on “View Surgical Visits” in the Activities tab. On this page, you will be able to see a list of all of your surgeries, including the date, time, location and patient name. You can also view a detailed summary of each surgery by clicking on the “View Summary” button
How to find your account information
If you have an account with Alliance Community Hospital, you can find your account information on the patient portal. To find your account information, log in to the patient portal and click on the "My Account" button in the top right corner of the screen. From here, you can view all of your account information, including your login name and password. If you have forgotten your login name or password, please contact their customer service team at (313) 863-6000.
How to request a health history
If you have been recently discharged from Alliance Community Hospital, please request a health history through their patient portal. The health history will help us keep you healthy and prevent future health problems.
To request your health history:
1. Log into the patient portal at www.alliancecommunityhospital.org/patient-portal/.
2. Click on the "My Health History" tab on the left-hand side of the screen.
3. Click on the "Request Health History" button in the "Health History Request Type" column.
4. Complete the health history form and click on the "Submit Health History Request" button.
