Welcome to the Affinity Birmingham Patient Portal! Here you will be able to access information and resources about your health, including your medical history, prescriptions, and more. To login, please enter your patient number (which can be found on your hospital identification card or on the registration form you received when you arrived at the hospital) into the login box below. If you have forgotten your patient number, you can also find it in their Patient Lookup section. Once you have logged in, you will be able to access all of the information in their Patient Portal. Affinity Birmingham Patient Portal – What are my patient account details?
If you are a registered patient at Affinity Birmingham, then your login details are as follows:
Log in to the Patient Portal using your Patient ID (found on your medical record) and password.
Once logged in, you will be taken to the main Patient Portal page. In the top right-hand corner of this page is a button which says ‘My Account’. Click on this button to access your account details.
On the My Account page, you will see a list of all of your previous appointments at Affinity Birmingham. Beneath each appointment, you will find information such as the date and time of your appointment, the type of appointment (e.g. consultation), and any notes that were made by their team about your visit.

If you want to keep your contact details up-to-date on the Affinity Birmingham Patient Portal, follow these simple steps:

3. Confirm your new password by clicking on the “Confirm New Password” button.
4. Click on the “Login” link at the top of the page to log in.
5. If you have forgotten your login ID, please enter your email address in the “Forgot Login ID?” field and click on the “Create Account” button.
