2. Enter your email address and password in the corresponding fields. Please note: If you have not created a password,societyhillOA will generate one for you. You will receive an email with this information.
3. Click on "Log In." Your account will be created and you will be able to access all of the features of the Patient Portal.
How to view your appointment history
If you have an appointment at Society Hill Ophthalmic Associates, you can view your appointment history on their Patient Portal. To login, click the "Log In" link on the top left corner of the homepage and enter your email address and password. You'll then be able to view all of your appointments, including the date, time, name of the doctor who treated you, and any prescribed medications or treatments.
How to adjust your appointment time
If you have an appointment that isn't working for you, please adjust your time as soon as possible. We understand that things can sometimes get busy, and we apologize if we are unable to accommodate your schedule. Please follow these steps to adjust your appointment time:
Step One: Log in to the website using your patient portal account.
Step Two: Click on "My Appointments" on the left-hand side of the screen.
Step Three: Look under "Appointments That Failed To Reschedule" and see if there is an open time slot that works better for you. If not, please click on the "Request Appointment" button and provide us with your new appointment date and time. They will do their best to accommodate you!
How to make a new appointment
If you need to make an appointment, the easiest way to do it is through their patient portal. Here's how:
1. Log in to the patient portal at www.societyhillocphthalmicassociates.com
2. Click on "My Appointments" under the "My Profile" heading on the left side of the page.
3. Click on the "Appointments" tab on the right side of the page.
4. On the "Appointments" tab, click on "New Appointment."
5. Complete the required fields and click on "Create Appointment."
6. You'll be taken to a confirmation page where you can click on "Accept Appointment." or "Cancel Appointment." If you accept your appointment, you'll be directed to their website for more information about your appointment time and location. If you cancel your appointment, you will be able to reschedule it at any time through their website or by calling their office.
How to add an alternate primary care physician
If you are not currently seeing your primary care physician through the Society Hill Ophthalmic Associates Patient Portal, you can add an alternate primary care physician. To do so, follow these simple steps:
1. Log in to the Patient Portal. If you have not already done so, create an account.
2. On the My Care Profile page, click on the Add a Primary Care Doctor link.
3. Enter your information and click Submit Form.
4. In the Alternate Primary Care Physician section, select your new primary care physician from the drop-down menu and click Save Changes.
5. You will now be redirected to your new primary care physician's profile page. Follow the instructions there to schedule future appointments and access your health records.
How to print or email your medical records
If you choose to print or email your medical records, follow these steps:
1. Log in to the Patient Portal.
2. Click the "My Medical Records" tab.
3. Click the "Print or Email My Records" button.
4. Follow the instructions on the next page.
