If you are a Sgrh patient, you can access your medical records from their Patient Portal. To login, please follow these steps:
1. Log in to the Patient Portal with your Patient ID and password.
2. Click on My Medical Records in the menu bar to view your medical records. You can also view your medical records by clicking on the My Profile link on the top right of the page.

How to Request a Copy of Your Medical Records
