To access the Patient Portal, please follow these steps:
1. Sign in to your MMH account at the Patients Connect site. If you do not have an MMH account, please create one before proceeding.
2. Click on the link that says "Patient Portal" in the top left corner of the Patients Connect home page.
3. On the Patient Portal home page, click on "Sign In."
4. Enter your username and password and click on "Sign In." You will now be authenticated in the Patient Portal.
The Patient Portal Homepage
Madison Memorial Hospital provides patients with a Patient Portal to stay connected to their medical information and health records. To login, patients will need their patient number and last name. Here's how to do it:
First, visit the Patient Portal homepage and enter your patient number in the search bar at the top. This will bring up all of your health information, including your medical history, lab results, and more!
Next, find your last name in the list of patients and enter it into the red box next to “Last Name.” If you don’t know your last name, you can find it on your hospital identification card or in your medical records. Once you have entered your information, click the “Login” button to continue.
You will now be taken to a page where you can log in using either your patient number or last name. If you have forgotten your patient number or last name, you can find them both on the Patient Portal homepage. Once you have logged in, you will be able to view all of your active health records and stay updated on changes to your medical information!

You will need your patient ID and password to login. If you have forgotten your password, please click the "Forgot Your Password" link at the bottom of the page and enter your patient ID and date of birth. You will then be able to reset your password.
Orders
If you are new to Madison Memorial Hospital, please use the following link to create an account: http://www.madisonmemorial.org/patient-portal/sign-in/. Once you have created your account, please follow these steps to order medications:
1) Log in to your account at www.madisonmemorial.org/patient-portal/sign-in/.
2) Click on “My Prescription History” at the top of the page.
3) On the right-hand side of the screen, click on “Order Medications”.
4) In the “Order Medications” box, you will need to enter the following information:
a) The name of the medication
b) The quantity of the medication
c) The pharmacy where you would like to order the medication
5) Click on “Submit Order”. Your order will be processed and a confirmation email will be sent to you. Please ensure that you check your spam folder if you do not receive this email within a few minutes.
