2. Enter your patient ID number or name in the login form on the homepage.
3. Click the "Create an Account" button to create a new account or log in if you are already registered.
4. Enter your email address and password in the appropriate fields and click the "Log In" button.
5. You will be taken to the main Patient Portal page where you can start managing your medical information and book appointments.

1. Go to https://www.linkedin.com/home
2. In the top right corner, click on your name (or your company's name if you're working for a company)
3. On the left hand side, click on "Account Settings"
4. On the right hand side, click on "Login"
5. Type in your email address and password, and hit "Enter"
Logging In with Facebook
In order to login to the Icanotes Patient Portal with Facebook, you will need to create an account on the Facebook website and then log in to your account. You can find instructions for logging in to your Facebook account here:

Once you have logged in to your Facebook account, click on the link called "Icanotes Patient Portal" which will take you to the Icanotes Patient Portal page. On this page, you will need to enter your user name and password in the appropriate fields and then click on the "Log In" button. You will then be taken to the Icanotes Patient Portal login screen where you can finish setting up your account.
How to Change Your Password
If you have forgotten your password, or if you would like to change it, please follow these steps:
1. Log in to the Icanotes Patient Portal using your username and password. If you have not registered for an account yet, you will need to do so first.
2. Click on the “My Account” link on the top navigation bar.
3. On the My Account page, click on the “Forgot Password?” link in the “My Profile” section.
4. Enter your username and email address in the appropriate fields and click on the “Create New Password” button.
5. You will be prompted to enter a new password twice. Make sure that you remember this new password and do not share it with anyone! Once you have entered a valid password, click on the “Save” button to update your profile information and reset your password.
How to Remove Yourself from a List
If you no longer want to receive email notifications from Icanotes, you can unsubscribe by clicking on the “unsubscribe” link in any email notification you receive.
Adding a New Authorized User
Adding a new authorized user is easy! In order to add a new user, follow these steps:
1. Navigate to the Icanotes Patient Portal main page (http://www.icanotes.com).
2. Click on the "Users" tab on the left-hand side of the page.
3. Click on the "Add User" button in the top-right corner of the screen.
4. Enter the user's full name and email address into the appropriate fields, and click on the "Create User" button.
5. Congratulations! The new user has now been added to Icanotes and can start using the patient portal immediately!
Deleting an Authorized User
If you need to delete an authorized user on the Icanotes Patient Portal, follow these steps:
1. Log in to the Icanotes Patient Portal.
2. Click on the "Authorized Users" tab.
3. Click on the "Delete Authorized User" button next to the authorized user you want to delete.
4. Click on the "Confirm Deletion" button.
