If you are a patient of Foothills Nephrology, you may have seen or used their patient portal. This is a great resource for keeping track of your health and medical history, seeing your doctors and view lab results etc. To login to the patient portal, follow these simple steps:
1) On the homepage of their website, click on “Patients” in the top navigation bar.
2) In the pop-up window that opens, enter your Patient ID number and click on “Log In”.
3) Enter your password and click on “Log In”.
4) You will now be taken to the main patient portal where you can access all of the information and services that we offer!
How to login to the Foothills Nephrology Patient Portal
If you are a patient with Nephrology appointments at Foothills Medical Center, you can login to the Patient Portal through your patient portal account. To create a patient portal account, please go to https://www.foothillsmedicalcenter.com/patients/login and click on the “Create My Account” button. After creating your account, you will be able to access all of your information in one place, including your appointment history and billing information. You will also be able to view important messages and updates from their team and make changes to your personal information.
How to find your health history on the Patient Portal
If you're new to the Patient Portal, or if you've forgotten your username and password, you can find instructions on how to login below. Once logged in, you'll be able to find your health history and see what medications you're currently taking.
Entering medical information into the Patient Portal
If you are a patient at Foothills Nephrology, you can use their Patient Portal to easily and securely manage your medical information. To login, follow these steps:
1. Log in using your patient number and password. If you have forgotten your password, click the “Forgot Password” link on the home page and enter your patient number and date of birth. You will then be emailed a new password.
2. Click the “My Medical Info” tab to view your medical history and current medications. You can also add or edit medical information here.
3. Click the “Communications & Appointments” tab to view upcoming appointments, communicate with your doctor, and view documents related to your appointment. You can also add or edit appointments here.
4. Click the “Payment Options” tab to view available payment methods and make payments online. You can also view insurance information and bills related to your care here.
5. Click the “Patient Reviews” tab to read reviews from other patients about their experience with their Patient Portal.
Managing your health records on the Patient Portal
The Foothills Nephrology Patient Portal is a secure website designed to help patients manage their health records. To log in, first click the "Login" link on the homepage. Enter your patient number and password, and click "Log In." You will then be taken to the "My Profile" page. On this page, you can access your personal information, medications, labs results, and more. You can also update your contact information and medical history. Requesting copies of your health records from the Foothills Nephrology Department
The Foothills Nephrology Patient Portal is a secure online system that allows patients to request copies of their health records from the department. To login and access the patient portal, follow these steps:
1. Log in to the Foothills Nephrology Patient Portal by clicking on the "Log In" link at the top of the homepage.
2. Enter your user name and password.
3. Select your record type from the drop-down menu next to "My Profile."
4. Click on the "Request Records" button.
5. Fill out the required information in the "Request for Records Form."
6. Click on the "Submit Request" button.
7. Your request will be processed and you will receive a notification email informing you of the results.
Applying for Medicare or Medicaid
If you are a Medicare or Medicaid patient and wish to access your health information through the Foothills Nephrology patient portal, please follow these steps:
1. Click on the “My Account” tab on the homepage of the Foothills Nephrology website.
2. Click on “Apply for Medicare or Medicaid”.
3. Complete the application form and submit it to your provincial health authority.
4. Once your application has been processed, you will receive an email notification from your provincial health authority confirming that your application has been received and is being reviewed. Please allow up to six weeks for processing.
5. Once your application has been approved, you will be able to login to the Foothills Nephrology patient portal and access all of your health information.
